Ages and Stages Questionnaire (ASQ)
Because your child’s first 5 years of life are so important, we want to help you provide the best start for your child!
As part of this service, ParentLink provides the Ages & Stages Questionnaires to help you keep track of your child’s development.
We look forward to your participation in this ASQ Developmental Screening program. Have fun completing these questionnaire(s) with your child, and make sure he or she is rested, fed, and ready to play before you try the activities!

ASQ-3 Questionnaire
The Ages & Stages Questionnaire or (ASQ-3) asks questions about your child’s overall development such as: communication, gross motor, fine motor, problem solving, and personal social skills. You will be asked to answer questions about some things your child can and cannot do. The (ASQ-3) can be given at each stage of development, up to 5.5 years. ASQ:SE-2 Questionnaire
The Ages and Stages: Social Emotional Questionnaire (ASQ:SE-2), asks questions about some of your child’s behaviors. This questionnaire includes questions about your child’s ability to: calm down, take direction and follow rules, communicate, perform daily activities (e.g., eating, sleeping), act independently, demonstrate feelings, and interact with others. The (ASQ:SE-2) can be given at each stage of development up to 6 years. Click here to take the ASQ Social-Emotional-2 Screening Questionnaire.

If these questionnaires show that your child is developing without concerns, we will provide some ideas about how to encourage your child’s future development, and provide the next questionnaire at the appropriate time.
If completing these questionnaires bring up any possible concerns, we can talk about information and resources that can help. Information will not be shared with other professionals or agencies without your consent.
